Occupational Therapist Jennifer Woerner Leads with Care

With more than two decades of experience in healthcare, Jennifer Woerner brings leadership and a collaborative spirit to Midland Care’s PACE program. As an occupational therapist and therapy team leader, she helps ensure participants receive coordinated, compassionate care that supports independence and quality of life.

Jennifer Woerner brings more than twenty years of experience and a deep commitment to teamwork to her role at Midland Care. She grew up near Emporia, Kansas, and began her healthcare journey early. While still in high school, she worked at a group home for people with developmental disabilities—an experience that sparked her lasting interest in caring for others and laid the foundation for a career centered on service.

Jennifer studied at Emporia State University, the University of Kansas, and the University of Kansas Medical Center. During college, she worked as a CNA in nursing homes and group homes. After becoming an occupational therapist, she spent nearly 20 years in home health before joining Midland Care’s PACE program.

Today, Jennifer serves as an Occupational Therapist and leads a team of physical and occupational therapists. She completes OT assessments and treatments while also coordinating communication and collaboration across the therapy team. She ensures PTs and OTs have the information they need to best support participants, joins morning meetings, helps problem-solve complex situations, participates in family meetings, and guides difficult decisions with compassion and clarity. She also works closely with her supervisor, Cheryl McIntosh, to coordinate medical respite discharges, helping PACE participants return home safely and efficiently.

Jennifer’s favorite part of working at Midland Care is being part of the strong PACE interdisciplinary team. She values shared decision-making and enjoys collaborating with social workers, nurses, and providers to support participants in a truly holistic way.

After years of working on the go in home health, Jennifer appreciates the daily benefits of being based at Midland Care. Having a regular workspace, seeing coworkers every day, and building consistent connections have made a meaningful difference. She is continually impressed by her colleagues and values the small, daily moments that strengthen trust and teamwork.

Outside of work, Jennifer’s love for the outdoors has grown into something special. She is an avid kayaker and hiker, and in 2014 co-founded Dirty Girl Adventures with her best friend. The organization leads outdoor trips—hiking, backpacking, camping, and kayaking—primarily for women, encouraging adventure, confidence, and community. Each year, Dirty Girl Adventures also travels to Guatemala to help provide healthcare to Indigenous Maya communities. Over four years, the group raised approximately $70,000 to support ODIM, the Organization for the Indigenous Maya—an accomplishment Jennifer is especially proud of.

Jennifer’s children, Cali (17) and Nate (14), are the most important part of her life, and watching them grow brings her the greatest joy. Jennifer describes herself as loyal and committed, always striving to do her best in every role she holds. Whether she is supporting PACE participants, leading her therapy team, or guiding outdoor adventures, her dedication is evident in everything she does.
