Community Aging in Place-Advancing Better Living for Elders (CAPABLE)
Stay Safe. Stay Independent. Stay home.
CAPABLE is a client-directed, home-based program that addresses both function and accessibility in your own home. The four to five-month program integrates services from an occupational therapist (OT), a registered nurse (RN), and a handy worker who work together to help the client set goals and implement strategies to improve health, independence, and safety.
CAPABLE is delivered in the home over 8-10 visits and is an evidence-based program
Research shows that older adults who can remain in their own homes feel better, report less pain, have decreased social isolation, and need fewer physician visits. Our goal is to help you age comfortably and safely in your home.
Occupational Therapist: An Occupational Therapist will complete a home safety assessment and work with the client to establish goals to help them perform self-care skills (such as dressing, grooming, bathing, meal prep, laundry) more independently and safely. The OT will also educate on home exercises for strength, balance, and mobility.
Registered Nurse: A Registered Nurse will assist the client in managing pain, answer questions about medications, and educate on fall prevention. The RN will collaborate with the client’s physician for urgent medical needs or medication/therapy adjustments.
HANDY WORKER: A Handy worker will collaborate with the OT and nurse to reduce hazards in the home by making minor home modifications such as grab bars, railings, toilet safety rails, and other safety features.

Who is eligible for Capable?
- Residents of Johnson County or Shawnee County
- 60 years of age or older
- Cognitively intact to participate in goal setting
- Own your home or have a landlord willing to have modifications completed
- Have difficulty with at least one daily activity, such as dressing, bathing, grooming, meal prep, or walking in or outside of home
- Individuals with certain medical conditions may not be eligible for this program
CAPABLE Improves Health Outcomes at Lower Costs. More than a decade of research shows that CAPABLE reduces the impact of functional impairment and disability, enhances motivation and self-efficacy, reduces health disparities, improves emotional health, and reduces hospitalizations and nursing home days.
CAPABLE is offered at no cost to participants through a partnership between Midland Care, Topeka Habitat for Humanity, Habitat for Humanity of Kansas City, and The Good Faith Network.
Program costs are supported through grants and are offset by reduced healthcare expenses, including fewer inpatient and outpatient services.
