Midland Care’s Program of All-inclusive Care for the Elderly (PACE) is a health care program that helps people at risk for needing nursing home stay in their own homes and communities.

PACE includes a team of experienced doctors, nurses and other health care professionals getting to know the participant and working with him or her to design a plan to aid in achieving health goals. PACE may be for you if you are interested in a team committed to helping you:

Be as healthy as possible for as long as possible
Remain in your own home and community with your family
Focus on what you can do, instead of what you cannot do

PACE includes health services:

doctor appointments
hospital visits
behavioral health services
dental and vision care
pharmacy
medical transportation
day center care
home support
nutrition services
medical supplies
nursing facility care when required

Participating in PACE simplifies access to health care and prevents fragmented health care that can be unsafe.

The History of PACE

PACE is an innovative program offered by over 100 local providers in 32 states, including Midland Care. The PACE model of care was created in 1973 in an effort to help the Asian-American community in San Francisco care for its elders in their own homes. For these families, the option of placing their elders in nursing homes was not a culturally acceptable solution. In order to meet this community need, local organizations created an innovative way to offer a comprehensive array of medical services using home care and an adult day setting. The model was tested nationally and found successful. PACE has been available in Northeast Kansas since 2007, when Midland Care opened the 37th PACE program in America. (1)

Am I Eligible for Midland Care PACE?

Midland Care PACE may be an option for anyone who:

Is 55 years old or over.
Lives in the Midland Care PACE service area in Wyandotte, Leavenworth, Shawnee, Douglas, Jackson, Jefferson, Osage, Pottawatomie, Wabaunsee, Marshall, Nemaha and Lyon counties in Kansas. If you live outside the Midland Care PACE service area, PACE may still be available from another provider. To find out, visit the National PACE Association online.
Qualifies for a nursing home level of care, meaning someone needs extra help to live at home safely, according to eligibility requirements determined by the State of Kansas.
Able to live safely in the community with the help of PACE services

How Can PACE Help?

PACE is a program designed to help you live independently and to meet your health and wellness goals. PACE participants work with their team of doctors, nurses, aides and other health care professionals to decide what kind of help they need most. Some of the benefits many PACE participants see include:

Less time in the hospital. PACE participants have fewer hospital admissions, hospital days and ER visits than people with similar conditions who are not enrolled in PACE (1)
Ability to live in your own home rather than a nursing home. PACE participants are 30% less likely to live in a nursing home. Most spend their entire lives in their own homes. (2)
A one-stop site for health care. Midland Care PACE has primary care, physical therapy, nutrition counseling, exercise classes, social activities and more all in one building. Medical transportation, including transportation to the PACE Center, is provided for you if you need it. You’re not on your own to get to appointments and find services you need. We can help.
Help at home. Midland Care PACE offers services in your home depending on need, including physical therapy, occupational therapy, assistance with personal care and even light housework. PACE also supports family caregivers with training, support groups and respite care.
A local team who gets to know you. The PACE team cares for a small number of patients so they can get to know you, your living situation, your family and your preferences. Midland Care PACE is a small, local, not-for-profit organization where you get personal attention.
24/7 support. A nurse is available to speak with PACE participants 24 hours a day, 365 days a year. We are always available to make sure your needs are met.

What does PACE cost?

What you pay for Midland Care PACE depends on your financial situation.

If you qualify for Medicaid and Medicare, you will either have a monthly payment or pay nothing for the long-term care portion of PACE depending on your income.
If you qualify for Medicare, all Medicare-covered services are paid for by Medicare. You will be charged a monthly premium to cover the long-term care portion of the PACE benefit and a premium for Medicare Part D drugs.
If you do not qualify for either Medicaid or Medicare, you will have a monthly payment for the long-term care portion of PACE depending on your income.

There is never a deductible or co-payment for any drug, service or care approved by the PACE team. Midland Care PACE is an approved provider of Medicare and Medicaid benefits.

By enrolling in Midland Care PACE, you agree to get all your health care from Midland Care PACE. Participants may be fully and personally liable for the costs of unauthorized or out-of-PACE program agreement services.
