Driven to Care: Taylor Zwygart’s Journey to Nursing
A determined nurse, dedicated mom, and lifelong learner who brings compassion and energy to the Midland Care PACE clinic.

At Midland Care PACE, compassionate care begins with dedicated team members who support participants every day. Taylor Zwygart, a Clinic LPN, is one of these essential team members. Since joining the PACE clinic just over a year ago, Taylor has become a valuable part of the care team, assisting participants with provider visits, addressing daily health needs, and building meaningful relationships along the way.
Taylor’s journey to Midland Care started in long-term care. Before joining PACE, she worked as an agency nurse in several nursing homes across Jefferson County and the Topeka area. Traveling between facilities provided her with a wide range of experiences and helped her develop the flexibility and clinical instincts that serve her well in a busy clinic environment.
She earned her Licensed Practical Nurse (LPN) credential from Washburn Tech in 2023. Even after entering the workforce, Taylor has continued to pursue her nursing goals. She is currently back in school working toward her Registered Nurse (RN) license, a step that reflects her determination to keep growing professionally while expanding the ways she can care for patients.

Today, Taylor works in the Midland Care PACE clinic, where no two days are the same. Her role ranges from rooming participants for provider appointments to helping triage participant needs in the day center. The pace and variety keep the work engaging, but what stands out most to Taylor is the opportunity to build relationships with the people she serves.
PACE Nurse Practitioner Paige Coffman says that commitment is easy to see in Taylor’s daily work.
“Taylor is not only an amazing nurse, but she also brings compassionate care to the participants she serves. Her day is not just spent in the clinic—you will often see her out in the day center talking with participants and triaging needs. Even if her day is busy, she will take moments out of her schedule to address their individual concerns while effectively communicating with providers to deliver excellent care.”
One of her favorite aspects of working at Midland Care is the supportive environment. Having a supervisor who encourages her growth, along with a team of coworkers who collaborate closely, makes a significant difference. Equally important are the relationships she forms with PACE participants, whose stories and personalities add a unique touch to each day.
Taylor also values the benefits that help employees care for themselves while caring for others, including Midland Care’s retirement match and “Taking Care of You” days.
Originally from Meriden, Kansas, Taylor graduated from Jefferson West High School and spent much of her early nursing career working throughout Jefferson County. Recently, she purchased a home in Topeka to be closer to work, which makes it easier for her to stay connected with her team and the participants she serves.
Outside of work, Taylor leads a busy and energetic life. She enjoys spending time with her daughter, whom she describes as her greatest inspiration and the motivation behind her ongoing education and career goals.
Her household is lively in other ways as well. Taylor has two kittens, Ahsoka and Espeon, a Labrador named Waylon, and a new Australian Shepherd puppy named Bailey. Between parenting and caring for her pets, there is rarely a dull moment.
When she finds time to relax, Taylor enjoys playing Pokémon. She has started collecting Pokémon cards and recently acquired a Game Boy to revisit some of the classic Pokémon games.
Taylor describes herself as determined, passionate, and a bit chaotic. However, those around her tend to see her as disciplined, hardworking, and motivated—qualities that help her balance work, school, and family while continuing to grow in her nursing career.
She’s also a sports fan who cheers for the KSU Wildcats and the Kansas City Chiefs.
For Taylor, the future is about moving forward—both for herself and her daughter. This determination, combined with her compassion for others, is part of what makes her such an important member of the Midland Care PACE team.
